Why the Difference Matters
The Women's Health Initiative study published in 2002 evaluated synthetic hormone therapy using Premarin and Provera. Those medications were associated with increased risks of breast cancer, stroke, blood clots, and cardiovascular disease.
Bioidentical estradiol is chemically identical to the estrogen naturally produced by the body, resulting in different metabolism and tissue interactions than synthetic hormone preparations.

Possible Side Effects

When appropriate, progesterone is commonly prescribed alongside estrogen to support endometrial health and create a balanced hormone replacement program.
Who Should Use Caution
Women with fibroids, fibrocystic breast disease, gallstones, elevated cardiovascular risk, or a history of blood clots require individualized treatment planning. Oral estrogen may not be the best option in these situations, and transdermal formulations may be considered instead.
Important Note
Women with active breast cancer should not receive estrogen therapy unless specifically directed by their oncology team.
